In this case of the week the authors present an interesting case of a large OS ASD with a large shunt fraction and significant tricuspid regurgitation. How valid is measurement of a tricuspid annulus dimension in the 4 chamber plane ( there is general thinking that the tricuspid valve predominantly enlarges in the AP direction explaining why Maslow et al (JCVA 2004; 18:719-24) found the trans- gastric RV inflow outflow window the most concordant with surgically measured dimensions of the valve. In a late presenting ASD increased RV after- load could also contribute potentially to TR. Lastly, as the authors point out shunt fraction calculations in patients with AV valve regurgitation is both tricky and tenuous because the errors at each stage tend to add up !
